Referrals to Our Practice

Carolina Asthma & Allergy Center is no longer using the Phreesia Platform (Charlotte Connect) or the Phreesia Dashboard. To better suit the needs of referring practices and our physicians and staff, we have made some changes.  These changes will continue to allow the referral process to be simple with sufficient communication and updates sent back to referring practices.

For Referring Practices & Referring Providers:

Please choose one of the three options listed below to refer a patient to Carolina Asthma & Allergy Center.*

1. Send referrals electronically to our direct address through EHR (This is the quickest, most efficient way.) To receive a copy of these addresses, please call our Referrals Department directly at 704-998-0866
2. Send a Fax directly to our Referrals Department at 704-644-1827
3. Call our Referrals department directly to refer a patient over the phone at 704-998-0866

*This is for all locations except for our Hickory location. To send a referral to our Hickory location, please fax your referral to 828-327-3223.

For Patients:
